Clayton county children and adults of all ages who do not qualify for Medicare or Medicaid, do not have private insurance and cannot afford to see a doctor.

P A T I E N T    I N F O R M A T I O N

QUALIFICATIONS and HOW TO MAKE APPOINTMENT:
You MUST be a resident of Clayton County.
You MUST not have Insurance Coverage (including Medicare/Medicaid).
Clinic services are for all ages.
(Walk-ins are difficult to accomodate - Please make an appointment)

WHAT TO BRING ON YOUR FIRST APPOINTMENT:
Bottles of your current medication.
Picture ID.
Social Security Card.
Interpreter (if possible), if you do not speak English.
Payroll Slip (or proof) if you work.
Current Utility Bill with your Name and Address (proof of residence).
Current Year's TAX Return.
Letter of Medicare/Medicaid or Social Security Denial.

  • Laboratory Tests
  • Women's Clinic - Pap Smears
  • Women's Clinic - Mammograms (over 40 years old)
  • Psychological Counseling
  • Evaluation by Selected Specialists
  • Pharmaceutical Patient Assistance Program Registration
  • Selected Surgical Skin Procedures
What Services are NOT Provided:
  • Dental Care
  • Narcotic Medication Prescriptions
  • Disability Evaluations
